ACKNOWLEDGEMENT OF RISK AND AGREEMENT: I consent to my child's or myself participation in this Event for Comunidad De Fe Ministries (CDFM). I agree on behalf of myself and my child, to assume all the risks in connection with my child's or my self’s participation in this event. I agree that my child or myself will follow relevant directions, rules, and policies and will obey directions. I acknowledge that Staff has the right to remove my child or myself from participation in this event.

HEALTH AND SAFETY: I know of no health-related reasons or problems which preclude or restrict my child or myself from participating in this event.

RELEASE OF LIABILITY: I agree TO RELEASE CDFM, its trustees, officers, employees, volunteers, members, and representatives from any and all liabilities and claims whatsoever arising out of, or in connection with, my child's or my self’s attendance and participation in this event.

MEDIA RELEASE: CDFM may make and use images, film, video, and audio recordings of my child or myself, and take and use quotes or statements from my child or myself, during the event, without compensation. I release all claims against CDFM with respect to privacy, copyright ownership, and publication, related to the use of the quotes, images, or recordings.

ACKNOWLEDGEMENT: By reading and continuing this legally binding document, I know I am voluntarily waiving the right to sue CDFM if my child or myself is injured while participating in this Event. In the event of my incapacity or death, this agreement binds my heirs, executors, administrators, and representatives.
